The Authors report a case of epidermoid cyst of the spleen observed in a young woman; this lesion is interesting on account of its clinical rarity and therapeutic implications. After a review of the literature they report the peculiar pathologic and clinical aspects of the cystic lesion and point out that diagnostic staging is founded on the use of TC and echography for excellent definition of morphologic and topographic patterns. In their opinion, splenectomy is the treatment of choice in relation to the morbility and mortality rate; on the contrary they deem a conservative treatment possible in patients, especially in pediatric children, at high risk of to severe post-splenectomy sepsis.
