Pulmonary embolism (PE) is a possible complication of right sided endocarditis in patients with large vegetations. In case of paradoxical atrial shunt a passage of septic emboli could induce a rare form of stroke. A case of tricuspid valve endocarditis, PE and cryptogenic stroke in an 81-year old woman without history of drug abuse and intra venous lines and with atrial septal aneurysm is described. The quick and fatal evolution did not permit exhaustive further examinations to detect a paradoxical shunt, but this remains a reasonable possibility. The present case demonstrates how rare complications of right side endocarditis, usually a relatively benign condition, could cause life-threatening complications.
