An English Cocker Spaniel was admitted for ambiguous external genitalia consisting in a penis-like structure bringing the urethral opening protruding from the vulva. Radiography confirmed the presence of an os penis; GnR stimulation test caused an increase of testosterone levels. Cytogenetic analysis provided a 78XX karyotype, and the search of SRY gene was negative. Internal genitalia, composed by a small uterus and two gonads resembling testes with epididymes, was removed by laparotomy. Histology confirmed the absence of ovarian tissue in the gonads and found two deferens running in parallel to the uterine horns. A diagnosis of SRY-negative XX male syndrome was made, focusing on the clinical approach and etiopathigenesis of the disorder.
