We report the case of a premature infant with respiratory distress syndrome and patent ductus arteriosus. The admission ECG showed a striking P-R segment elevation (up to 2 mm) in leads I, II, III, aVF, V4 and V6. T-troponn was increased, and the echocardiogram showed 2 left atrial thrombi. 7 days later, the P-R segment elevation disappeared and intracardiac thrombi were reduced in size.
