The authors report a case of haemoperitoneum as an initial sign of post-traumatic intraperitoneal rupture of hepatic hydatid cyst. They stress how this condition, not previously described in the literature, put diagnostic doubts for the unusual clinical presentation and for the US and CT images suggestive of liver parenchymal lesion. The patient underwent emergency surgery and a total pericystectomy, by US guidance, was performed. The authors emphasize that total pericystectomy is the most efficient surgical technique in the treatment of complicated hepatic hydatid cysts.
