There are two phases following head trauma: primary and secondary injury. Primary injury (concussion, contusion, laceration) refers to pathologic changes that take place at the time when injury occurs. Secondary injury refers to a variety of pathologic processes which develop after the primary insult, resulting in progression and extension of cerebral damage. Hypotension and cerebral ischemia are both factors that have a significant impact on the pathogenesis of secondary injury. They induce autolysis of brain tissue and intracranial hypertension which can be potentially fatal. Treatment should focus on limiting the development of secondary injury by restoring of cerebral perfusion and oxygenation and reducing cerebral edema. The prognosis depends on: severity of primary damage, location of injury and access to immediate managment
