Kawasaki's disease is an acute systemic febrile illness of unknown aetiology, mainly affecting children younger than five years of age. Generally the prognosis is favorable but delayed diagnosis and treatment with intra venous immunoglobulins can represent a risk factor for the severe complications of this disease, which include coronary aneurysm and myocardial infarction. Atypical cases of Kawasaki's disease are common (up to 10% of cases) and consequently the diagnosis should be considered even when not all of the diagnostic criteria are met. The Authors report on two children, aged 3 years and 7 months, who presented fever and thrombocytopenia associated with scrotal swelling at the onset of the illness. A timely and appropriate immunoglobulin therapy rapidly normalized the temperature and the other clinical signs of the disease, thus avoiding cardiovascular complication. The authors conclude that the presence of hydrocele in a febrile child with thrombocytopenia may represent an early clinical sign of Kawasaki's disease and justify a timely commencement of intra venous immuno globulin therapy.
