Aim of this paper is to describe the unusual epididymal localization of a testicular adrenal rest tumor in an adolescent affected by salt-wasting congenital adrenal hyperplasia. Multiple hypoechoic lesions involving both testes and epididymis were demonstrated with multiparametric ultrasonography and magnetic resonance imaging.
