Freeman-Sheldon Syndrome (FSS), recently reclassified as distal arthrogryposis type 2, is a rare congenital syndrome characterized by skeletal malformations, craniofacial deformities and progressive myopathy causing multiple body contractures and respiratory muscles involvement. The literature concerning about this syndrome focuses mostly on pediatric patients. We present the anesthetic management of a forty-year old male patient affected by FSS undergoing elective wedge pulmonary left resection via muscle-sparing mini-thoracotomy. Written informed consent was obtained for this report.
