On the basis of the more recent literature on aneurysmal SAH the present situation and problems, as well as the future prospects of pathophysiological and experimental research are reviewed. Cerebral vasospasm remains the major hazard for both morbidity and mortality in SAH and microvascular BBB changes represent an important additional factor in the occurrence of the global brain dysfunction post-SAH. Different experimental models have been developed in several species: despite efforts to clarify the cellular and molecular events underlying vasospasm and BBB changes by in vivo models, the pathomechanisms remain unclear. Recent advances in cellular and molecular biology and better understanding of genetic and biochemical bases of SAH will made gene therapy of SAH consequences a realistic goal.
