Bilateral painful ophthalmoplegia is usually related to a secondary etiology and cannot be diagnosed as Tolosa-Hunt Syndrome according to the ICHD criteria. Few idiopathic cases with bilateral complaints were reported. We describe a patient with bilateral, relapsing -remitting painful ophthalmoplegia co-occurring with Hashimoto’s thyroiditis and hyperhomocysteinemia in which laboratory and MRI investigations failed to detect any causative pathology.

Abstract
